Every resident (or resident family member) and employee will have to fill out a form to be eligible for the COVID-19 Vaccine. Verbal consent may be given for a family member at CTKM and an employee can fill out the form information.

 CTKM is receiving the Pfizer vaccine.

All consent forms must be handed back in no later than 12/29/2020 at noon.
